To be accepted to study at EAC, interested applicants must go to the Admission/Guidance Office.

Applications may be obtained by calling at 416-4341 / 416-4342 local 106; by writing and/or visiting the Admission/Guidance Office if you have any questions or if you need additional information

Admission Requirements and Procedures

STEP 1	Proceed to Admission/Guidance Office for inquiries and list of requirements
STEP 2	For incoming FRESHMAN:
	Pay College Entrance Test (CET) Fee to the Cashier Submit Official Receipt (O.R.) to Admission/Guidance Office Fill-up and submit copy of Application form with recent I.D. pictures attached Take the entrance exam in the Admission/Guidance Office
	For TRANSFEREES/SECOND COURSES:
	Refer to nos. 1-4 for incoming FRESHMEN Submit copy of grades (graded class cards); or Transcript of Records for evaluation of respective Deans
STEP 3	Get the result of the CET after one (1) hour and secure a medical referral slip from the Admission/Guidance Office
STEP 4	Submit all the requirements to the Admission/Guidance Office
STEP 5	Proceed to Enrollment Area at Lobby 2 for Manual Advising by faculty advisers
	For OLD STUDENTS (REGULAR/IRREGULAR):
	Proceed to respective colleges for Manual Advising to get subjects to be enrolled Present FINAL GRADE REPORT
STEP 6	Go to the Cashiers Office for payment of Tuition and Medical Exam Fee
STEP 7	Secure Registration Form from Registrar’s Office and ask for request slip for school I.D.
STEP 8	Proceed to Information Technology Center (ITC) for I.D. Picture Taking
STEP 9	Proceed to the School Clinic for Medical/Physical Exam
STEP 10	Avail the School Uniform at EAC Bookstore

Enrollment Requirements

	FRESHMEN (NEW STUDENTS):
	1. Form 138 (High School Report Card) - Original Copy 2. Certificate of Good Moral Character - Original Copy 3. 3 pcs. 2x2 Recent ID Pictures (Any Background) 4. 3 pcs. Long Brown Envelope
	Other Requirements can be submitted before the end of the semester:
	5. Birth Certificate from NSO (xerox copy) 6. Physical Exam Result and O.R.
	TRANSFEREES/SECOND COURSES
	1. Copy of Grade (graded class cards)/Transcript of Records for evaluation 2. Honorable Dismissal from previous (recently attended school) - Original copy 3. Refer to nos. 3-6 for FRESHMEN
	EAC RETURNEES (who drops out from studies):
	Re-activate student number – Registrar’s Office Those student who stopped 1 to 2 years will be given same student number Those student who stopped 3 years up will be given another student number Those who will shift their course from Non-Para to Paramedic Course will be given another Student number Proceed to respective school for manual advising (Get subjects to be enrolled)
